are there any installation and configuration instructions for this utility?

No install is necessary.  Just download spwakeup.exe and copy it to your MOSS/WSS server.
I've tried to make it as easy as possible. If you run the program without any command line options it uses the most popular configuration.  It will search the local machine for Web Apps, then search the Web Apps for Site Collections and Sites.  Once it has found everything, it will send an HTTP request to each site.
That's the default action of just double-clicking on the file, or running it from the command line.

If you need to change some of the defaults, you can add command line switches, for example, -log:c:\logs\log.txt would tell the app to log the results to a file instead of just displaying them on the screen.  If you run spwakeup.exe -help it will tell you what all the various options are.
